Job Summary

In order to support the key priority of creating healthy communities by reducing wait times for specialist care, the Ministry of Health and Long-Term Care (MOHLTC) is supporting the expansion of an electronic referral (eReferral) system to South East LHIN. The program delivering the eReferral technology, program and overall framework is referred to as System Coordinated Access.

Some LHINs across the province have been selected to act as the local delivery partner of the program and will work with the SCA program and local stakeholders to facilitate the adoption of the eReferral solution to ensure that local and provincial requirements and needs are being identified and addressed. The South East LHIN will act as the local delivery partner and will work with the SCA program and local stakeholders to facilitate the adoption of the eReferral solution to ensure that local requirements and needs are being identified and addressed.

The System Coordinated Access (SCA) Program Office, based out of the eHealth Centre of Excellence in the Waterloo Wellington LHIN will provide provincial oversight and coordination to support local delivery partners in the deployment of the eReferral solution with an initial focus on the orthopedic referral pathway.

The Opportunity

You will have the rare opportunity to play a key role in the successful implementation and adoption of the eReferral technology in the South East LHIN. You will apply a structured change management approach and methodology to support primary care and specialists in the adoption of the eReferral technology in a variety of healthcare settings. The selected candidate will need to build and support numerous stakeholder relationships and work with and learn from change management supports within the SCA program to ensure a successful implementation in South East LHIN.

Responsibilities and Duties

As the Change Management Specialist, you will:

  • Be a primary point of contact for primary care physicians and specialists using the eReferral solution in South East LHIN including solo practitioners, family health teams, family health organizations, and others
  • Work with the SCA Program Office Change Management Lead to apply a structured change management approach and methodology to support eReferral adoption
  • Work with the SCA Program Office Benefits Realization Lead to produce reporting and data analytics to support the overall adoption of eReferral
  • Provide on-job support and formal training to primary care physicians and specialists
  • Work closely with primary care and specialists to strengthen their capacity on e-referral, data analysis and data use
  • Be an active and visible coach to clinicians and stakeholders
  • Work with primary care, specialists and the SCA program team to create workflows and manage measurement systems to track adoption, utilization and proficiency of users
  • Identify potential risks, points of resistance, performance gaps and develop appropriate mitigation activities. Work with partners to remove barriers to change
  • Work with the SCA Benefits Realization program team to support evaluation to capture the organizational and clinical value of SCA and eReferral
  • Maintain information exchange and collaboration with the SCA Program Change Management Lead and Change Management team to ensure local referral strengthening activities complement the broader program vision
  • Support SCA participation agreement sign off
  • Document lessons learned and present findings in a logical and easy-to-understand manner
  • Create and enable reinforcement mechanisms and celebrations of successes

Qualifications and Skills

The candidate we seek will have a profile that includes:

  • University degree in Healthcare Administration, Business Administration or a related discipline
  • Change Management Professional certification preferred
  • Minimum of 5 years of experience supporting digital health initiatives or within a health care capacity
  • Previous experience with enabling technologies and digital health within Ontario
  • Experience in data reporting and management
  • Prior experience in evaluation or research an asset
  • Strong communication skills (written and oral)
  • Excellent active listening and interpersonal skills
  • Strong ability to pay attention to detail
  • Excellent relationship building, collaboration, and engagement skills, including conflict resolution, negotiation and mediation
  • Ability to manage frequent demands of multiple stakeholders
  • Able to work effectively with a diverse group of clinical and non-clinical stakeholders
  • Ability to work in a diverse and fluid working environment recognizing that different opinions and backgrounds can bring strength to the tasks at hand
  • Demonstrated experience quickly building and maintaining effective and productive working relationships in complex, multi-stakeholder healthcare environments
  • A solid understanding of clinical processes (primary, acute and community care)
  • Well-developed understanding of the impact of technology on patient service delivery
  • Proficient in SharePoint, Microsoft Office, Microsoft Project
  • Knowledge of provincial and local health-care priorities, initiative, and issues.
  • Knowledge of technologies employed in primary healthcare an asset
  • Knowledge of South East LHIN business strategies, objectives, priorities and programs, and related communications priorities and plans
  • Knowledge and understanding of the South East LHIN’s stakeholders as well as overall issues and priorities within the healthcare sector
  • Detailed knowledge of South East LHIN administrative practices, policies and procedures, especially around confidentiality and release of information
